When visiting us for the first time, be sure to bring along the following documentation:

All relevant personal information about the patient
All relevant information about the person responsible for the account
ID book (or copy) of the person responsible for the account
Consent for the appointment, from the person responsible for the account if he/she is not present
Medical aid card (if applicable)
Relevant medical history – provide medical reports if available
Referral letter from the dentist (if provided)
Records from the referring dentist (if provided)
Consultation Fee
Registration documents (You do a pre registration, this is very   
helpful and time saving – we can either fax or post the forms to you, you can    
complete them in your own time; alternatively; you can complete the  
necessary documents in our waiting rooms. Please take your time and make
sure that the information  you provide us with is accurate and legible)
Diagnostic records if any (Records that have been provided by the referring    
 dentist that are inadequate or outdated will be taken again)
